If by "huge cash cow" you mean "running interfacility transfers," then I totally agree. I don't think there's an ambulance company that operates a 911 service at a profit of that magnitude.

I don't know about that paddles one. I'm sure someone out there is still running around with a lifepak 5 or something. Some of the hospital Zoll's still have paddles as well.

Recently, my dad broke his back while tree-climbing. He's 62. I saw the x-rays of the fracture, and I've (and my dad have) known people who've never walked again after this kind of injury. Needless to say, within a month after the surgery, my dad has been making a recovery that's beyond what anyone in his medical team has expected, and he was up walking around (with assistance) within a few days of the surgery.

My sister lived in Denmark for a while, and told me that Falck/Falk (?) is somewhat similar to Rural/Metro over here. They provided a lot of EMS and Fire services to (I think) the majority of the country. She didn't have anything bad to say about them, but that's about the extent of my knowledge.

Diaphoresis, chest pain, and nausea along with Bradycardia might be suggestive that that heart rate (even though it's not the lowest Bradycardia I've ever seen) may be part of the problem.

O2, IV, 12-lead, I'd get. ASA perhaps, NTG perhaps...but first:

What was she doing prior to onset of symptoms? I'd start to consider getting out the Atropine if she gets worse, and if her story is suggestive of that being the underlying problem, but I'd want to know a lot more first. Associated difficulty breathing (and which started first, if at all), lung sounds and if pain/pressure is modified by palpation and deep inspiration.

d/d: Right MI, Other MI, Bradycardia, Pneumonia (if supported by lung sounds and changes to pain), Heartburn
